To enroll your child in our Pre K 3 program, your child must turn 3 years old by December 1st. All enrollment forms are available to print. Please print and complete the Preschool Registration Contract, our Toileting Policy, and the Extended Day Care Services Form (if needed). A $100 non-refundable registration fee is due upon enrollment. The registration fee can be paid by cash, check made out to St. Lawrence ECLC, or online through WeShare using the link below.
Once you are registered, you should receive an enrollment packet during the summer. All forms need to be completed and returned to ECLC by August 15th. Children are not permitted to start school without a completed NYS Child in Care Medical form and the NYS Daycare Enrollment form.

The NYS Child in Care Medical form must be filled out, signed by a licensed physician or nurse practitioner and dated within the past 12 months. You must include an immunization record along with the form. Some doctor’s offices use their own forms and immunization listings. You do not need to have them fill out the NYS form as long as the exam and immunizations are signed and dated. You can attach these documents to the NYS Form noting “see attached”.
